The elbow in arthrogryposis.

چکیده

Over 100 cases of arthrogryposis have been seen at the Royal Children’s Hospital in Melbourne in the past fifteen years and most of the patients are still under our care. This condition, which is characterised by multiple rigid joint deformities, occurs in families with an impeccable genetic background and there is no evidence of hereditary causation. The infants are born to young and older mothers alike. Extrinsic factors seem to play no part. The extent of affection varies from one or two joints to almost total involvement. Even the joints of the spine and the mandible may be implicated. Occasionally there is associated webbing or dislocation of involved joints, and spina bifida cystica may also be present. The nature of the affliction is not well understood but it is essential to appreciate the pathology in order to plan a logical scheme of treatment.

Posterior elbow capsulotomy with triceps lengthening for treatment of elbow extension contracture in children with arthrogryposis.

BACKGROUND Flexion of one elbow is essential to enable children with arthrogryposis to achieve independent function such as self-feeding and self-care of the face and hair. We analyzed the outcomes of posterior elbow capsulotomy with triceps lengthening for the treatment of elbow extension contractures in a series of children with arthrogryposis multiplex congenita. METHODS The medical record...

Pectoralis major transfer for paralysis of elbow flexion in children.

Seven pectoralis major transfers in children suffering from bilateral paralysis of elbow flexion due to arthrogryposis or to trauma are reported. A technique is described in which the muscle is mobilised from the clavicle to allow the tendon of insertion to be attached to the biceps tendon at the elbow. The biceps tendon was found to be present and could be mobilised forwards in all the arthrog...

Free muscle transfer for challenging and rare upper limb disorders

Free innervated muscle transfer (FMT) achieves functional restoration following not only traumatic brachial plexus injury, but also intractable rare upper limb disorders, such Hopkins’ syndrome, myoplasia in arthrogryposis and radition neuropathy. I introduce our current experience of FMT for these disorders. Hopkins syndrome is a rare cause of poliomyelitis-like paralysis affecting one or more...

Arguments for a neuroorthopaedic strategy in upper limb arthrogryposis

We present two children with a diagnosis of upper limb arthrogryposis and report on findings about brachial plexus exploration and a nerve transfer procedure to reanimate elbow flexion. Although the etiology of arthrogryposis multiplex congenita remains unknown and multifactorial, it can be worthful to explore the brachial plexus in the affected upper limb and to perform selective motor nerve t...

Transfer of pectoralis major in arthrogryposis to restore elbow flexion: deteriorating results in the long term.

We present the long-term results of pectoralis major transfer to restore elbow flexion in seven patients (ten procedures). The early results in all the patients were encouraging but with longer follow-up a gradual and progressive flexion deformity was observed with a decrease in the arc of flexion in eight elbows, reaching > or = 90 degrees in all cases. The results of pectoralis major transfer...
